Intake / Eligibility

Eligibility:

Adults living with significant mental health issues requiring support and over 16 years of age.

Area Served:

Broadview Avenue east to Warden Avenue, and Lakeshore Boulevard north to Eglinton Avenue, in Toronto (see map).

Referral Process:

We are part of the centralized intake process with The Access Point. Most mental health and addictions programs in the city now have The Access Point as the first step of the intake process. You can reach them at 1-888-640-1934 or www.theaccesspoint.ca to complete an application.
